Waitlist Sign Up There is a $125 non-refundable fee to be added to the Waitlist, payable via Venmo.

Medication Authorization This form is required before we will administer any medication to your child.

Tuition Express You have the option to set up fee-free Autopay with your bank through Tuition Express, or you can use a credit card which includes an additional 3% convenience fee per transaction.

How to Fill Out the Form

Select the form you would like to fill out from the forms above. Download the PDF to your computer. Open the file in Adobe Acrobat Reader.

Once you open the PDF file in Adobe, multiple options will be listed on the right-hand side of the menu bar. Select “Fill & Sign” if you prefer to type in your responses. For signatures, you may select the pen icon along the top menu bar and then select a font preference.

You can also simply print a hard copy and fill it out manually and sign the document.

Submit the Application

Email the filled & signed electronic form or scan the signed form that you manually printed and filled out, and send the form to Compass Academy at compassacademyga@gmail.com
